Reminder: Health Forms Due December 1

As a reminder, health history forms are due to the NYU Health Center by December 1st.

NYU students should have picked these up at 726 Broadway, 3rd floor.  Please plan to do so ASAP if you have not already.

Visiting students should have received these at their permanent address in early to mid-November.  If you were admitted on or after October 31st you can expect these to arrive next week.  Please return them to the Health Center as instructed at your earliest convenience.
